Flemingdon Health Centre is a fully accredited, non-profit
charitable community health centre (CHC) located in
Toronto, Ontario serving our clients within a defined
geographic area. Funded by the Ontario Ministry of Health
and Long Term Care (MOHLTC), we are governed by a
volunteer Board of Directors consisting primarily of residents
and/or clients. This is one way the community has a say in
identifying health needs and how to meet them.

 

 

 

 

We provide a wide range of primary health services with an emphasis on illness prevention, health promotion, health education and community development. We employ salaried health professionals including doctors, nurses, social workers, chiropodists, a nutritionist, an x-ray technologist, and community health workers. Flemingdon Health Centre is responsive to the needs of our diverse community. Our staff speak a number of languages and we are able to link with other agencies to provide interpretation and translation services.

The Centre works in partnership with a variety of health organizations to ensure continuity of care. To address the broad determinants of health we also work with community partners such as schools, legal clinics, neighborhood & social services, and grass roots community organizations. Additionally, through our teaching relationship with the University of Toronto, we provide quality up-to-date services and programs.

Working with local residents, we initiate social action and community development activities to address issues such as unemployment, housing, and isolation. History as a (CHC)

Staff at community health centres do more than treat sick people. They offer programs and services to keep people healthy. Flemingdon Health Centre originated in 1972 as a primary health centre on Don Mills Rd, and was originally operated by the Sunnybrook Hospital University of Toronto Clinic.

There are over 50 CHC's in Ontario. These centres treat health problems and also offer programs and services to keep people healthy. There is a rigorous accreditation process for CHC's. Flemingdon Health Centre completed the process in January 2008 and is fully accredited.

General Information

Do I have to Pay? (Public Insurance)

Services at Flemingdon Health Centre are covered by the province's health care system. When visiting the health centre you will be asked to present your Ontario Health Card. This card shows that you are entitled to the Ontario Health Insurance Plan, or OHIP. Show this card each time you use medical services.

OHIP pays for most necessary medical services such as visits to a family doctor, hospital or clinic, and laboratory tests. OHIP does not pay for dental care (except surgery done in a hospital), eye glasses or contact lenses, and prescription drugs.

What are my rights in health care?

All FHC clients will be treated with respect, dignity and consideration regardless of race, sex, citizenship or financial status.

You have the Right to:
- Confidentiality
- Understand your medical treatment
- Have your records transferred
- Refuse treatment

Focusing on the factors that affect health, Flemingdon Health Centre works with our clients and the diverse community to meet their primary health care needs.  Committed to health promotion and illness prevention, we enable social action initiatives and community development.  Through our affiliation with the academic community, we participate in teaching and other academic activities.

The values of Flemingdon Health Centre are consistent with the Pan American Health Organization (PAHO) /World Health Organization’s (WHO) Declaration of Montevideo representatives from all WHO member countries met in Montevideo, South America and reaffirmed the importance of equity and the right of every human being to enjoy the highest attainable standard of health without distinction of race, religion, political belief, economic or social condition. The declaration came into effect September 2005.

FHC Privacy Statement

As your health centre, all staff are bound by law and ethics to safeguard your privacy and confidentiality of your personal information.

This includes:
- collecting only the information that may be necessary for your care;
- keeping accurate and up-to-date records;
- safeguarding the medical records in our possession;
- sharing information with other health-care providers and organizations on a “need to know” basis where
  required for your health care;
- disclosing information to third parties only with your express consent, or when necessary for legal reasons;
  and
- retaining/destroying records in accordance with the law.

Your request for care from us implies consent for our collection, use and disclosure of your personal information for purposes related to your care. As noted above, other purposes require your express consent. You have the right to see your records. You may also obtain copies of your records – please see the receptionist for our fees for this service.

If you would like to discuss our privacy policy in more detail, or have specific questions or complaints about how your information is handled, please speak to the Executive Director and we will try to resolve them.*

Health Centre Hours: Monday to Thursday: 08:30 am to 09:30 pm Friday: 08:30 am to 06:00 pm Saturday: 09:00 am to 02:00 pm. A Doctor is always on call 24 hours.
