Becoming a registered client

In order to become a registered client, you must live within one of our catchment areas. If you live outside of this area, please visit the Association of Ontario Health Centres to find a Community Health Centre near you.

If you live within our catchment area, please call to book an intake appointment. Intake appointments are cannot be booked through email or fax. All adults over the age of 18 must attend the intake appointment. At the time of the intake appointment, new clients must bring:

  • Proof of address – any document that states your name and address (e.g. driver license, lease agreement, utility bill, etc.)
  • OHIP (Ontario Health Insurance) Card or proof of Permanent Residency.
  • Interim Federal Health Program Document (IFH) for refugee claimants.

Each adult over 18 must present a proof of address that states his/her specific name. A spouse’s proof of address will not be accepted. Children under 18 may use their parent’s proof of address.

Our services are open to all residents in the community. If you do not have some of these documents but live in our catchment area, please call us to determine your eligibilty for our services.

We are not a walk-in clinic and are only able to provide services to registered clients by appointment. Every effort is made to register eligible clients to our practice. When our registration is full and we are unable to accept new clients, your names will be added to a wait list and we will provide you with information and referral to various other resources in the community.

Please note: The first appointment to register will not involve meeting with a doctor. Appointments with health care providers will be booked following the intake appointment.
