Don Mills Diabetes Education Program

Who We Are

Don Mills Diabetes Education Program is a FREE program, consisting of a multidisciplinary team of registered nurses and dietitians, program coordinator, community outreach worker and support staff serving clients living with Type 2 diabetes. The program is funded through the Ministry of Health/Toronto Central Local Health Integration Network (TC-LHIN). No matter where you live, you are welcome at the diabetes program.

What We Offer

  • Group Sessions: weekly workshops on general diabetes management, conducted by the nurse and dietitian in a relaxed and friendly atmosphere. Family and friends welcome! Not a drop in program, please call to register.
  • Individual Counselling: meet with the nurse and dietitian to learn how to live well with type 2 diabetes and maintain a safe blood sugar level. Self-referrals are welcome.
  • Special Workshops: food label reading, carbohydrate counting, physical activity, cooking, safe fasting during Ramadan, as well as regular review sessions.

Our staff offers group sessions and individual counselling in English, Urdu, Hindi, Spanish, Cantonese and Mandarin. Free interpretation is available for most other languages. Our sessions are offered in the morning, afternoon or evening in small groups at convenient locations in the community.

What We Talk About

Managing diabetes is different for everyone. Our team will work with you to manage your diabetes in ways that fit into your life. Topics could include:

  • healthy eating and meal planning
  • active living
  • testing blood sugar (receive a FREE glucometer if you need one)
  • coping with stress
  • foot care
  • other relevant topics

How to attend

If you are interested in coming to our centre, you can call and book your own appointment. While a doctor’s referral is not necessary, asking your doctor to complete this referral form will provide us with useful information.

What happens at the appointment

Please arrive 15 minutes early for your first appointment to complete the registration form. Please bring a copy of your medications and your blood glucose meter if you have one.

Initial appointments are 2 hours — 1 hour with the nurse and 1 hour with the dietitian. Follow-up appointments will be booked as needed.
