2022-2027 Strategic Plan

This strategic plan has been developed in the middle of a global pandemic which has disproportionately impacted our local communities. We wanted to harness the experiences and insights that have emerged through this time to help shape our path forward.

2024-2025 FHC Annual Report

The theme for our 2024-2025 Annual Report is: Your Health, Our Commitment: Expanding Access and Improving Outcomes Together. In this report you’ll see stories of innovation, community-driven care, and partnerships that shaped the year, from new models of primary care and creative wellness programs to the opening of the Thorncliffe Park Community Hub and our 50th anniversary celebrations.

The theme for our 2022-2023 Annual Report is: A Collaborative Journey Towards Health & Well-being. Uniting Community, Inspiring Integration. Throughout this report you will hear about the ways in which FHC have collaborated and innovated to improve health outcomes for our community members and strengthened connections with other community based organization.

2021-2022 FHC Annual Report

We chose to focus our 2021-2022 Annual Report on ‘Planting for the Future.’ We anticipate that the next phase of our work will include nurturing and supporting the regrowth and re-energizing of our staff, communities and other stakeholders. All of us will have a role in rebuilding our health care landscape and ensuring that the post pandemic world is one we are proud to call home.
