Get involved to support our vision of Strong Healthy Communities

Strong Healthy Communities are places where individuals and families feel a sense of belonging to something bigger than themselves. They are places where community members come together to address social determinants of health like the environment, housing, employment and opportunities for learning.

Donate

Flemingdon Health Centre is a Registered Charity. Your donations help us to provide accessible programs and services that are responsive to the diverse needs of our community.

Community Grants Program

  • The Community Project Grant provides up to $1,000 in one-time funds to small community groups to carry out projects that will benefit the communities served by Flemingdon Health Centre (O’Connor to Steeles; Victoria Park to Bayview) that might not otherwise happen.
  • Registered Charities or incorporated not-for-profit organizations are not eligible to receive this grant.  This year, we are specifically supporting applications that will support Black and Indigenous communities in our catchment area.
  • Successful projects are eligible to receive an amount up to $1,000.  The maximum amount to be awarded is $1,000. The project must be carried out within a year of the award of the Grant.  The announcement of the successful applicant were made on 31st Oct 2021.
