Our primary care programs and services are provided by an interdisciplinary team of health professionals. We provide our clients with available information and guidance to support them in the decisions they make about their health care. We prioritize clients who experience access barriers to health care and community services.

  • Services are FREE of charge and open to residents living in our catchment area.
  • OHIP coverage is NOT required.
  • Over-the-phone language interpretation services are also available as required for all services.

We do not offer emergency or walk-in services.

Medical Care

We offer client-centred health care services by putting our clients at the centre of any planning and decision-making about their treatment.

Virtual Care

With the help of live video and audio, our clients can now interface with their health care provider from the comfort of their own home.

We provide free, confidential 24-hour on-call assistance from staff physicians and residents.

We offer preventative health services to help our clients prevent illness, disease and other health-related problems.

Preventative services include:

  • Check-ups
  • Counselling
  • Screenings

Preventive measures can be applied at all stages across the lifespan and along a disease spectrum, to prevent further decline over time.

We offer health education services to help our clients understand illness, disease and other health-related problems.

Our teams support clients to ask questions, understand and act on the health information they have been provided in order to improve their present and long-term health.

FREE Sexual Health Clinic Toronto
Sexual Health Clinic

In partnership with Toronto Public Health, our Sexual Health Clinic provides:

  • Drop-in, confidential and non-judgmental sexual health information and counselling
  • Family planning services, such as pregnancy testing, birth control and free condoms
  • Testing for sexually transmitted infections (STIs)
  • Testing for HIV
  • Screening for cervical cancer through a Pap test

Dental Services

Toronto Public Health Dental offers FREE dental services at our Fairview Site to eligible children and youth (0-17 years), adults enrolled in select Toronto Public Health programs and seniors (over 65).

To be eligible for Dental Services:

  • You must live in Toronto
  • You do not have dental insurance
  • You cannot afford dental services

Medical Foot Care
Chiropody (Medical Foot Care)
  • Assessment and diagnosis of diseases, disorders and dysfunctions of the foot
  • Assisted care of non-curable soft tissue aliments
  • Minor surgical and soft tissue procedures
  • Fitting and manufacturing custom-made devices and braces (*may include associated fees)
  • Wound care services
  • Preventative care and education for overall foot health management
  • Entrance Criteria:
    • Priority to children under age 18 and seniors above 75 years of age;
    • Intake for individuals 18-75 is determined based on medical necessity and financial ability;
    • Does not include individuals with private insurance, motor vehicle accident assurance, WSIB etc
  • Clients are requested to fax referrals from their medical doctors and updated bloodwork at 416 422 3573 (Attn: Chiropody)
