OHIP coverage is NOT required.

We prioritize clients who experience access barriers to primary care and community services.

What is an Intake Appointment?

The intake appointment does not involve meeting with a doctor, but is required to complete your registration. At the time of the intake appointment, new clients must bring:

Proof of address – any document that states your name and address (e.g. driver’s licence, lease agreement, utility bill, etc.)
Proof of Permanent Residency
Interim Federal Health Program Document (IFH) for refugee claimants.

Every effort is made to register eligible clients to our practice. If we are unable to accept you as a new client, we will provide you with information and referrals to various other resources in the community.
