What is an Intake Appointment?

The intake appointment does not involve meeting with a doctor, but is required to complete your registration. At the time of the intake appointment, new clients must bring:

  • Proof of address – any document that states your name and address (e.g. driver’s licence, lease agreement, utility bill, etc.)
  • Proof of Permanent Residency
  • Interim Federal Health Program Document (IFH) for refugee claimants.

Every effort is made to register eligible clients to our practice. If we are unable to accept you as a new client, we will provide you with information and referrals to various other resources in the community.
