Please note that all FHC locations will be closed Monday, September 7, 2026 for Labour Day. Flemingdon Site (10 Gateway Blvd.) will be open Saturday, September 5 until 2 p.m. Fairview Site and Health Access Thorncliffe Park (HATP) will be closed September 5-7. The health care landscape is undergoing a period of significant change and renewal.

A key component of this transformation is the creation of the new Ontario Health Teams, which are part of a new model of organizing and delivering services in local communities. As a result, North York Toronto Health Partners was formed to fulfill the mandate of providing integrated health care for a defined patient population within our community.

While the goal of these teams is to care for entire populations, North York Toronto Health Partners will start by focusing on patient groups with more complex and higher needs, including frail seniors, those with mental health and addiction issues, and those at end of life.
