Our services and programs are free of charge and available to registered client who live within our catchment area.

You do not need OHIP coverage to receive care. We especially aim to support individuals who may face barriers in accessing care and provide language interpretation services.

What Is an Intake Appointment? 

An intake appointment is the first step in your registration. It does not involve meeting with a doctor.

During this appointment, please bring:

  • Proof of address (e.g., driver’s licence, 
lease agreement, utility bill)
  • Proof of Permanent Residency
  • Interim Federal Health Program (IFH) document for refugee claimants
  • Any past medical records
